* Root Cause
According to the Master’s explanations, during housekeeping and securing inside bosun’s stores, crew carried out sweeping and mopping of the floor, however, they observed that the old rubber matting is quite small so they decided to remove it to replace with a new one but forgot to put back in place the new rubber insulation matting. There were also tonner bags and hold cleaning materials that were placed near the switch board which was not been noticed.

* Corrections undertaken
Upon pointed out by PSCO, immediately removed the obstructions and placed back the rubber insulation matting, however the crew completed the rectification works when the PSCO already disembarked the ship so the deficiency has remained pending despite of the mentioned corrections.
