* Root Cause
According to the Master’s explanations, all the crew particularly the deck personnel were observing the wearing of proper PPE when working around the mooring stations. However, warning notices obliging this safe practice were not posted to notify all persons onboard. Safety inspection is regularly conducted by the Safety Officer which includes confirming the availability of safety notices posted.
These required warning notices/markings are to be confirmed by the Safety Officer during monthly safety inspection. Although it was not stated in the Master’s report, we have speculated that the absence of warning notices at the two mooring stations might be due to the following causes:
* The original warning symbol marking painted on deck could have been either covered by deck paint during painting maintenance (if the warning notice were stenciled at the mooring station, or, the warning notice have already faded out and was not been restored.
* The warning symbol stickers (in case stickers were used instead of stenciled marking) may have deteriorated or blown away by strong winds but was not been replaced with new one.
* Checking the availability of these symbol mark was overlooked because it was not specifically stated in the Checklist for Safety Officer’s monthly inspection.
* Confirmation of above warning notice was not included in the Risk Assessment for mooring operation to continuously remind all crewmembers about this obligation for personal safety.
* Corrections
To rectify this deficiency, Master immediately instructed the OIC to post-up the OSHA-compliant safety signs such as the wearing of helmets, safety boots, googles, hand gloves, etc. to each mooring stations and took photos as evidence.
