* Root Cause

* According to the Master’s explanations, Due to vessel stayed 12 days at anchorage and 3 days in port they generate an average of 0.01m3 per day of food waste (total accumulated food waste of about 0.15m3). To prevent foul smell on deck, Master decided to keep food waste inside the fish room after double-bagging and sealing, and this is based on Master’s experience on other port (i.e. Australia) where authorities allowed to put the food waste inside the reefer chamber as long as the bag containing food waste is kept sealed and no leak.
*  No prior consultation with local agent if authorities in Canada allow the option of storing the food waste inside the cold provision reefer or not, for obtaining advanced information.
*  The procedures stated in the Garbage Management Plan (GMP) Section 2.3.6 pertaining to storage of food waste was not been applied appropriately.


* Corrections undertaken
The Company instructed the Master that the food waste must be removed from fish room, put it on sealed container bin, spray with sodium hypochlorite for disinfection and minimize foul smells. After departure, when vessel more than 12NM to the nearest land, it will be disposed overboard in compliance with MARPOL regulation. The fish room is thoroughly cleaned and brief all personnel onboard not to keep the food waste in provision store/ reefer room.
